Screening
Instructions:
Blood Pressure
- Scoring: 1 point for each entry of systolic value; 1 point for each entry of diastolic value.
- To submit your blood pressure, enter your systolic, or upper value, in the top field and your diastolic, or lower value on the bottom field.
Screening Tests
- Choose to enter data in any or all of the categories.
- Scoring: 1 point for each exam.
- For PSA test, using the drop down menu, enter exam’s month and day in top fields and the year in the lower field.
- For Mammogram, using the drop down menu, enter exam’s month and day in top fields and the year in the lower field.
- For Prostate, using the drop down menu, enter exam’s month and day in top fields and the year in the lower field.
- For Vision, using the drop down menu, enter exam’s month and day in top fields and the year in the lower field.
- For Pap Smear, using the drop down menu, enter exam’s month and day in top fields and the year in the lower field.
